At Last Resort Medical Transport, we are committed to providing non-emergency medical transportation that caters to your needs. Our trained drivers are kind and compassionate and specialize in medical appointments and hospital discharges. Our service is available in Oshkosh, Omro, Fox Valley and Appleton areas only.
Rates for scheduled rides:
WHEELCHAIR AND AMBULATORY RATES ARE THE SAME
* We will contact you with the estimated fare once the ride is confirmed. We do not provide an ADA-compliant accessible vehicle at the moment.
PICK UP FEE
$30.00
Each time a passenger loads the vehicle.
MILEAGE FEE
$2.75
The fee charged per mile from pick-up address to drop-off address.
WAIT FEE
$11.00
If the driver has to wait for the passenger, there is a fee for each 30 minutes of wait time from the start of the scheduled appointment time until the passenger loads the vehicle.
LATE CANCELLATION FEE
$50
Cancellations made within 24 hours of the scheduled pickup time.
NOTE:
PASSENGERS REQUIRING A WHEELCHAIR MUST BE ABLE TO STAND AND TRANSFER INTO THE VEHICLE SAFELY.
Last Resort Medical Rides is not authorized to bill insurance. We accept payment from private pay individuals via cash, check, debit/credit card. *If you are an individual that works with one of the above, please make sure to have contact information if payment will be made via one of these sources. ALL RETURNED CHECKS WILL BE CHARGED A $50 NSF FEE.

Our service is available 24/7 for short notice rides for a flat fee of $120 (Oshkosh) and $160 (Neenah,Appleton,Omro,Menasha
Serving Oshkosh, Omro, Fox Valley and Appleton areas only
Nights, weekends and holidays are considered after hours.
All rides must be confirmed by both parties via phone or text prior to pick up. Call us for any questions.
